Procedure Details

This patient presented with a diagnosis of right breast cancer and a history of laparoscopic gastric banding (Lap-Band) for weight loss. After considering her options, the patient chose to have bilateral mastectomy and immediate breast reconstruction with innervated DIEP flaps using the ReSensation technique. This approach was a good option to address the significant hanging abdominal skin and residual fat deposits associated with her weight loss surgery. The patient underwent 2 subsequent outpatient surgeries to revise the reconstructed breasts and abdominal donor site. Revisions included mastopexy to lift the reconstructed breasts, liposuction, fat grafting to smooth breast contours, and scar revision. GalaForm 3D, a bioresorbable mesh, was also inserted as an internal bra to provide long-term soft tissue support to maintain the lifted breast position. The patient completed her reconstruction with nipple-mound reconstruction and tattooing.

Treating the Whole Patient

This patient benefited from using reconstructive techniques for breast reconstruction that also addressed excess, loose skin that developed after weight loss surgery. This approach delivered a dramatic improvement in both her breast and abdominal appearance. When reconstruction was complete, the patient expressed that she felt and looked better than she had prior to her breast cancer diagnosis.
