This nice lady in her 60's had breast augmentation with silicone implants 30 years ago. Her annual screening mammogram identified rupture of both implants. The patient complained of gradual hardening and superior migration of her implants. This was associated with breast discomfort. She presented to Albright Plastic Surgery for surgical treatment of her ruptured silicone breast implants and painful capsular contracture. Removal of the ruptured implants with a total capsulectomy was recommended. The patient also chose to have new implants placed with a breast lift to achieve a more youthful breast appearance. One of her goals was to decrease her breast size from 38 D cup to a C cup. A vertical breast lift technique was used with removal of additional breast tissue to achieve the desired reduction in breast size. New 275 cc smooth round Mentor MemoryGel moderate classic profile silicone gel implants were placed after cleaning out all the old silicone material and capsule tissues with a total capsulectomy. The patient healed well from her revision augmentation-mastopexy ("aug-pexy") surgery as seen in her 1 year postoperative photos.
